Ichthyophthiriasis is a very common parasitic disease in carp and many other freshwater fish. It is caused by a tiny single-celled ciliated parasite called Ichthyophthirius multifiliis, which settles under the skin and gills of the fish. This disease is also known as white spot disease because of the small white grains that appear on the animal's body. It affects both farmed carp and ornamental carp (koi) in ponds and aquariums.

The most characteristic sign is the appearance of numerous tiny white spots, resembling grains of semolina or salt, on the skin, fins, and gills. Affected fish rub themselves against the pond walls, rocks, or bottom, as if scratching. Erratic swimming patterns are often seen, with fish remaining still near the surface or alternatively swimming in an agitated manner. Breathing may become more rapid if the gills are affected, with fish coming to the surface to gulp air. In advanced cases, appetite decreases, the fish becomes lethargic, and the fins may appear damaged or frayed.

Causes and risk factors
The disease is caused by a ciliated parasite, Ichthyophthirius multifiliis, which develops in several stages. The adult parasite, called a trophont, lives embedded under the skin or in the gills of the fish, forming the visible white spots. After a growth phase, it leaves the fish, sinks to the bottom of the water, and forms a capsule in which it multiplies into very many small free-swimming cells called theronts. These theronts swim actively through the water looking for a new fish to infect, which explains the rapid spread in a pond or basin.
Cool to mild water temperatures, typical of spring or autumn conditions, particularly favour parasite development. Overstocking of ponds or tanks greatly increases the risk of transmission between fish. Introduction of new fish without a quarantine period is a common cause of disease appearing in a previously unaffected population. Stress from transport, poor water quality, sudden temperature changes, or unsuitable diet weakens the natural defences of carp and makes infestation more likely.
Transmission
The parasite spreads directly from one fish to another through the water via its free-swimming forms. It does not require an intermediate host. Contamination can also occur through introduction of infected fish, soiled equipment, or water from a contaminated pond.
Diagnosis and care
Diagnosis is the veterinarian's responsibility, based on clinical examination and laboratory tests. Care is decided case by case.
A veterinarian or aquaculture specialist diagnoses the condition by observing the characteristic white spots on the fish's body. Microscopic examination of a sample of skin mucus or a fragment of fin or gill confirms the presence of the parasite and distinguishes it from other skin diseases. Assessment of water conditions (temperature, quality) and farming practices completes the evaluation.
Treatment
Treatment is mainly based on antiparasitic products suitable for fish, applied to the pond water according to professional guidance. Improving farming conditions (water quality, temperature, stocking density) is integral to managing the disease. Treatment must target the free-living forms of the parasite in the water, as forms embedded in the fish's skin are largely inaccessible to treatments. It is essential to consult a veterinarian or aquaculture specialist to choose a product appropriate for carp and its intended use (food production, ornamental), as some treatments may be toxic or unsuitable depending on the context.

Prevention
Systematic quarantine of new fish before adding them to a pond or tank is the most effective measure. Maintaining good water quality, avoiding overstocking, and minimising stress for the fish are important. Regular monitoring of fish behaviour and appearance allows early detection of disease, before it becomes widespread.
Possible complications
Heavy infestation can cause serious damage to the gills, leading to severe breathing difficulties. Skin lesions can lead to secondary infections, particularly bacterial or fungal. In severe or untreated cases, mortality can be substantial, especially in young fish or with high stocking densities.
