Body inclusion disease is a serious viral disease that affects snakes, including some species of sea snakes. It is caused by an arenavirus which settles permanently in the body of the infected animal. The name of the disease comes from the presence, inside the cells of many organs, of small structures called "inclusion bodies" which can be observed under a microscope. This disease most often develops chronically and progressively, with marked neurological damage which ultimately seriously affects the animal's quality of life. It is mainly documented in snakes in captivity (boas, pythons) but similar cases or risks are mentioned in certain sea snakes, particularly during contact with captive populations or in collection contexts.

The signs of the disease are variable and often appear gradually. The most characteristic sign is a neurological disorder: the animal may present a loss of balance, an inability to turn around correctly, unusual head movements, or a sort of "spin" of the body without apparent cause. We also often observe a progressive loss of appetite, weight loss, general lethargy and sometimes repeated regurgitation after meals. Some animals develop seizures or partial paralysis in later stages. The signs may remain discreet for a long time before becoming significantly worse.

Causes and risk factors

The disease is caused by arenaviruses, a family of viruses that particularly infect snakes (sometimes called "reptarenavirus"). The virus replicates in the cells of many organs and causes the formation of characteristic structures called inclusion bodies, visible inside cells during laboratory tests. These inclusion bodies are the signature of the disease and give the condition its name.

The risk factors identified in snakes mainly concern life in captivity and in collections: contact with carrier animals often without visible symptoms, presence of external parasites such as mites which could participate in transmission, high density of animals in the same space, and absence of quarantine when new individuals are introduced. Specifically for sea snakes, data remains limited and the risk is mainly mentioned in the event of contact with captive collections.

Transmission

Transmission would mainly occur through direct contact between snakes (secretions, bites) as well as through external parasites such as snake mites which could transmit the virus from one animal to another. Transmission through contaminated equipment or surfaces is also suspected. Transmission from mother to offspring is not clearly established but cannot be excluded.

Diagnosis and care

Diagnosis is the veterinarian's responsibility, based on clinical examination and laboratory tests. Care is decided case by case.

The diagnosis is based on clinical observation by a veterinarian specializing in exotic animals or reptiles. The veterinarian looks for characteristic neurological signs and asks about the history (recent introduction of animals, contact with other snakes). Additional analyzes may be offered: search for inclusion bodies on organ biopsies (liver, kidneys, esophagus), blood tests, molecular biology tests looking for viral genetic material (arenavirus), and post-mortem examinations in the event of death to confirm the diagnosis. No 100% reliable rapid test exists in current practice; definitive diagnosis often requires specialized laboratory tests.

Treatment

There is no specific antiviral treatment recognized for this disease. The care is only supportive: comfort care, nutritional assistance if necessary, management of the environment (temperature, humidity) to limit stress, and treatment of possible secondary opportunistic infections with suitable therapeutic families (antibiotics, antifungals) on veterinary advice. In very advanced cases with significant suffering, a discussion on the animal's quality of life with the veterinarian is recommended.

Treatment depends on the cause and the animal's condition: it must be determined by a veterinarian. No self-medication.

Prevention

There is no vaccine against this disease. Prevention is essentially based on biosecurity measures: strict quarantine of any new animal before bringing it into contact with other snakes, rigorous control of external parasites such as mites which could play a role in transmission, disinfection of equipment and living spaces, and avoidance of unnecessary contact between animals of different origins. For wild sea snakes, the main risk concerned is linked to contact with captive populations or uncontrolled releases.

Possible complications

Once the neurological signs have set in, the disease most often progresses towards progressive worsening. Possible complications include complete inability to feed or move, repeated seizures, complete paralysis and death. Secondary immunosuppression can also promote other opportunistic infections (bacterial, fungal or parasitic) which complicate the clinical picture.

The snake loses its balance or is no longer able to turn around normally.

The animal exhibits abnormal head movements or a sort of body twisting.

A loss of appetite or weight loss sets in without an obvious cause.

Repeated regurgitation occurs after meals.

Convulsions or partial paralysis appear.

A new animal introduced into a collection shows abnormal behavior.

Prognosis

The prognosis is poor once the clinical signs, particularly neurological, are established. The disease generally progresses chronically over several months, with a progressive deterioration in general condition. There is no known curative treatment to date; the treatment aims to improve the comfort of the animal.
