Dirofilariasis is a parasitic disease caused by a roundworm, Dirofilaria immitis, which settles in the right heart and pulmonary arteries of the infected animal.\n\nIn the Tibetan fox, a wild canid living in the highlands of Central Asia, this infection can occur where the parasite and its mosquito vector are present, particularly in areas where domestic dogs, stray dogs and wildlife coexist.\n\nThe adult worm can measure several tens of centimeters and live for several years in the body, causing progressive wear and tear on the heart and lungs.

Causes and risk factors
The disease is caused by a nematode, Dirofilaria immitis, transmitted exclusively by the bite of mosquitoes.\n\nThe mosquito ingests microscopic larvae (microfilariae) present in the blood of an already infected animal, the larvae then develop inside it, then are deposited in the skin of a new host during a subsequent bite.\n\nThe larvae then migrate into the tissues before entering the bloodstream and settle, several months later, in the right heart and the pulmonary arteries where they become adults.
The main contributing factors are the presence of mosquito vectors, proximity to domestic or stray dogs carrying the parasite, a hot and humid climate favoring mosquitoes, and prolonged outdoor exposure without any antiparasitic protection.\n\nThe wild life of the Tibetan fox, in contact with other canids and exposed to mosquito bites in its natural environment, can constitute an exposure factor depending on the geographical area.
Transmission
Transmission occurs only by the bite of a mosquito vector; there is no direct contagion between animals.
Diagnosis and care
Diagnosis is the veterinarian's responsibility, based on clinical examination and laboratory tests. Care is decided case by case.
Veterinary diagnosis is based on specific blood tests looking for either circulating larvae (microfilariae) or antigens of the adult parasite.\n\nAdditional examinations such as chest x-rays or cardiac ultrasound make it possible to assess the impact on the heart and lungs.

Prevention
Prevention is essentially based on the fight against mosquitoes and the use of preventive antiparasitics adapted to canines, under veterinary supervision.\n\nIn a wild animal like the Tibetan fox, individual prevention is not applicable; rather, surveillance involves controlling the populations of domestic and stray dogs in areas of contact with wildlife, in order to limit the reservoir of the parasite.
Possible complications
Without treatment, the disease can progress to right heart failure, pulmonary hypertension and coagulation disorders.\n\nThe sudden death of worms in the vessels, particularly after certain poorly supervised treatments, can cause potentially fatal pulmonary embolisms.
