Simian malaria, also called malaria, is a parasitic blood disease. It affects great apes, including chimpanzees, and can also infect humans under certain conditions. It is caused by microscopic parasites called Plasmodium, transmitted through mosquito bites. In chimpanzees, several Plasmodium species closely related to those found in humans occur naturally, particularly in wild populations in Africa. This disease is closely monitored because it illustrates parasite exchange between great apes and humans.

Signs vary considerably depending on the Plasmodium species involved and the animal's general condition. Intermittent fever is often seen, along with marked depression and loss of appetite. The animal may appear very tired and refuse to move normally.
In more severe cases, the mucous membranes (gums, lips, inside of mouth) become pale due to destruction of red blood cells by the parasite. A yellowish discoloration of the mucous membranes or skin (jaundice) may appear. Vomiting and progressive weight loss are possible.
In some severe cases, particularly in young or weakened animals, seizures or neurological disease may occur. Many infected chimpanzees remain without visible symptoms for long periods, with the parasite circulating quietly in the blood.

Causes and risk factors
The disease is caused by single-celled parasites of the genus Plasmodium. In chimpanzees, several species are found, some of which are genetically very close to the Plasmodium species responsible for human malaria. The parasite invades and destroys red blood cells, which explains the tiredness and paleness observed.
Living in an area where Anopheles mosquitoes are abundant, particularly in humid tropical forests, increases the risk of exposure. Young animals, weakened individuals, those that are sick or stressed, and those living in dense groups appear more vulnerable to symptomatic forms. The presence of other great apes or infected humans nearby can sustain parasite circulation through mosquitoes.
Transmission
The parasite is transmitted through the bite of Anopheles mosquitoes, which act as vectors. There is no direct transmission from one animal to another without the mosquito. The parasite cycle involves a phase in the mosquito followed by a phase in the blood and liver of the chimpanzee.
Diagnosis and care
Diagnosis is the veterinarian's responsibility, based on clinical examination and laboratory tests. Care is decided case by case.
The veterinarian suspects the disease when unexplained fever is present along with pale mucous membranes, especially in a high-risk area. Diagnosis is confirmed by microscopic examination of a blood drop (blood smear) that shows the parasite inside red blood cells. More sensitive tests, such as molecular biology analysis, can be used in a specialized laboratory to precisely identify the species of Plasmodium involved.
Treatment
Treatment relies on antiparasitic medications targeting Plasmodium, selected by the veterinarian depending on the species involved and the severity. General supportive care may be needed in cases of marked anemia or dehydration, such as fluid therapy. No treatment should be given without veterinary advice, as the choice of drug depends on the situation and possible drug resistance.

Prevention
Prevention relies mainly on mosquito control: mosquito nets, suitable repellents, environmental management to limit insect breeding areas. In care facilities or sanctuaries, protecting enclosures with mosquito nets is an important measure.
Regular veterinary monitoring with blood tests allows detection of infection before severe symptoms appear. There is no vaccine available for this disease in chimpanzees.
Possible complications
Possible complications include severe anemia from massive destruction of red blood cells, brain involvement (cerebral malaria) in severe forms, or failure of multiple organs. In an already weakened animal, infection can worsen an underlying disease and complicate overall management.

However, people in close contact with wild or captive chimpanzees in tropical areas should remain cautious and protect themselves from mosquito bites.
Prognosis
The prognosis depends strongly on the Plasmodium species involved, the speed of treatment initiation, and the animal's overall health status. Many infections remain mild and well tolerated, especially in healthy adults. Severe forms, particularly in young animals or weakened individuals, can be life-threatening if not treated promptly. Early intervention significantly improves the chances of recovery.
