Neonatal diarrhea is the leading cause of loss of calves in the first weeks of life, in Tunisia as elsewhere. It is not a single disease but a syndrome: several different microbes cause the same picture.
What kills the calf is almost never the microbe itself: it is the loss of water and mineral salts in the stool. A calf that empties becomes dehydrated within a few hours.
Two elements decide the fate of the calf well before the appearance of liquid stools: the quantity and quality of colostrum drunk at birth, and the cleanliness of the calving room.

A calf that still looks “fine” but whose stools are already liquid loses water with each pass. Waiting to see him in bed is treating it too late.

These signs are neither exhaustive nor specific. Only a veterinarian can establish a diagnosis.

Liquid, abundant stools, yellow, whitish, greenish or brownish depending on the agent in question. Color alone does not allow you to name the microbe.
Dirty hindquarters and tail, strong odor.
The calf suckles less quickly, then refuses the bucket or the feeding.
Signs of dehydration: sunken eyes, skin on the neck that remains wrinkled when pinched, dry and cold mouth.
Gradual decline: the calf remains lying down, no longer follows its mother, no longer reacts when someone enters.
In severe cases: inability to get up, head resting on its side, body cold in the extremities.
The fever is inconstant; a very dehydrated calf may even be below normal temperature, which is a sign of seriousness and not healing.
Blood or pieces of mucous membrane in the stools point to certain causes and require rapid veterinary advice.
When to consult without delay
This list does not replace veterinary advice. It gathers situations that justify a prompt consultation.
Causes and risk factors
Several families of agents are responsible, often associated in the same calf:
Bacteria, mainly certain strains of Escherichia coli, and sometimes salmonella.
Viruses, especially rotaviruses and coronaviruses.
Microscopic parasites, cryptosporidia.
Coccidia in slightly older calves.
These agents are present in almost all livestock farms. They only cause disease when the infection pressure is high (dirty building, high density) or the calf's defense is weak (insufficient colostrum, cold, stress).
An error in milk feeding — milk that is too cold, too concentrated, distributed irregularly — aggravates or triggers diarrhea without any particular microbe.
Late, insufficient or poor quality colostrum intake: this is the number one factor.
Calving in a dirty, damp box, used without cleaning between two cows.
Difficult calving: a calf that suffered at birth sucks poorly and absorbs antibodies less well.
Concentration of calvings over a short period, with calves of different ages housed together.
Cold, drafts, damp litter.
Young or poorly nourished mother at the end of gestation, whose colostrum is poorer.
Poorly washed buckets, teats and probes between calves.
Transmission
The agents are excreted in the feces of sick calves and carrier adult animals, which show no signs.
Contamination occurs through the mouth: soiled udder, dirty litter, poorly cleaned buckets and teats, breeder's hands and boots, equipment passed from one box to another.
A sick calf excretes enormous quantities of infectious agents: it contaminates the box, then the calves born after it. The pressure thus increases throughout the calving season.
Diagnosis and care
Diagnosis is the veterinarian's responsibility, based on clinical examination and laboratory tests. Care is decided case by case.
The veterinarian first assesses the degree of dehydration: position of the eyes in the socket, folds of skin, warmth of the extremities, sucking strength, ability to stand. It is this assessment, and not the color of the stool, that decides the urgency.
He looks for other possible sources in a slaughtered calf: navel, joints, lungs.
Stool tests can identify the causative agent. They are especially useful at the herd level: knowing the dominant agent helps guide prevention and vaccination of mothers for subsequent calvings.
In the event of repeated mortality, an autopsy and samples provide answers that the examination of living calves does not provide.
Treatment
The absolute priority is rehydration, started as early as possible: it is this which saves the calf, before any treatment directed against the microbe. The veterinarian indicates the appropriate rehydration solution, how to prepare it and the rate of distribution.
A calf which is no longer suckling, which is no longer standing or which is cold requires an infusion by the veterinarian: the liquid given by mouth is no longer absorbed.
Breastfeeding should not be stopped without advice: the calf needs energy to fight. The veterinarian specifies how to distribute milk and rehydration solution.
Antibiotics are not systematic. They have no effect on viruses and cryptosporidia, and their indiscriminate use maintains resistance. They are reserved for situations where the veterinarian suspects the passage of bacteria into the blood.
Supportive treatments may be prescribed: anti-inflammatory, digestive protectants, specific products against cryptosporidia.
The sick calf must be isolated, kept dry and warm, on abundant bedding.
Treatment depends on the cause and the animal's condition: it must be determined by a veterinarian. No self-medication.
Prevention
Give good quality colostrum as soon as possible after birth, without waiting for the calf to fend for itself. The absorption capacity of antibodies by the intestine decreases hour after hour and disappears within a day; the veterinarian indicates the volume and rhythm adapted to the calf.
Check that the mother has usable colostrum and provide a frozen reserve from healthy cows from the farm in case she is short of it.
Calve in a clean, mulched, disinfected box between two cows; clean the udder before the first feeding.
House the calves individually for the first few days when possible, and strictly separate the sick from the healthy.
Follow a simple work order: take care of the youngest calves first, the sick last.
Wash and disinfect buckets, teats and probes after each use; never pass the same material from a sick calf to a healthy calf without cleaning.
Distribute milk at constant temperature and concentration, at regular times.
Protect from cold and humidity: dry litter, absence of drafts on the ground.
At the end of the calving season, empty, clean and allow the premises to dry before the following season.
Vaccination
Vaccines for mothers exist against the main agents of neonatal diarrhea, including rotaviruses, coronaviruses and certain strains of Escherichia coli.
We do not vaccinate the calf: we vaccinate the pregnant cow, at the end of gestation, to enrich its colostrum with antibodies.
This principle has a decisive practical consequence: the vaccine only protects if the calf actually drinks this colostrum, early and in sufficient quantity. A well-done vaccination with poorly distributed colostrum is of no use.
No vaccine covers cryptosporidia. The veterinarian chooses the vaccination based on the agents identified in the farm and sets the time of the injection.
Possible complications
Severe dehydration and acidification of the blood, which explains depression and death.
Passage of bacteria into the blood, with damage to the joints, navel or meninges.
In calves that survive: lasting growth retardation, dull coat, increased susceptibility to respiratory diseases in the months that follow.
Diarrhea that persists can lead to poor digestion of milk for several weeks.
When to see a veterinarian
The calf has liquid stools and sucks less well: call the same day, do not wait until the next day.
His eyes appear sunken, the skin on his neck remains wrinkled when pinched: dehydration has already set in.
He no longer gets up, his mouth is cold, he no longer reacts: it's an emergency.
There is blood in the stools.
Several calves are affected in a few days: it is a herd problem, to be treated as such.
A calf dies despite treatment: request an autopsy to protect those following it.
Transmission to humans
Some of the responsible agents — cryptosporidia and salmonella in particular — can infect humans and cause diarrhea.
Wash your hands thoroughly with soap after each contact with a sick calf, its stall or equipment, and before eating.
Wear clothing and boots intended for livestock use.
Keep young children away from diarrheal calves; people with weakened immune systems should avoid all contact.
Prognosis
A calf treated from the first liquid stools, rehydrated early, most often recovers without after-effects.
The prognosis quickly darkens as soon as the calf can no longer stand: every hour lost counts.
At the farm level, results depend less on treatment than on colostrum management and calving hygiene.
Frequently asked questions
Should we stop milk when the calf has diarrhea?
No, not from yourself. The calf needs the energy from milk to fight, and depriving it weakens it. The veterinarian indicates how to distribute the milk and the rehydration solution throughout the day depending on the calf's condition.
Does the color of the stool tell us what the calf is suffering from?
No. Yellow, white or greenish, the color does not designate a specific microbe. Only a stool analysis answers this question, and it is especially useful to guide prevention for the entire herd.
Does an antibiotic cure calf diarrhea?
Not in most cases. Viruses and cryptosporidia are not sensitive to it, and a given antibiotic systematically promotes the appearance of resistance. It is the veterinarian who judges whether it is justified.
Only one calf is sick: should I worry about the others?
Yes. A diarrheic calf releases enormous quantities of germs and contaminates the premises. Isolate him, deal with him last, and clean the equipment before returning to the healthy calves.
Is vaccinating cows enough to prevent diarrhea?
No. The vaccine acts through colostrum: if the calf does not drink it quickly enough or not enough, protection never occurs. Vaccination complements good colostrum management, it does not replace it.
