Simian malaria, or primate malaria, is a disease caused by microscopic parasites of the genus Plasmodium. These parasites live in the blood and are transmitted through mosquito bites. In capuchins, this disease mainly affects animals living in tropical environments or in contact with areas where infected mosquitoes are present, such as certain regions of Central and South America where the species originates. In captivity, outside these areas, the risk is very low but exists if carrier mosquitoes are present.

Signs are often subtle at first. Fever, marked tiredness and loss of appetite may be seen. The animal may become depressed, less active, and gradually lose weight. The mucous membranes (gums, inside of lips) may become pale due to destruction of red blood cells by the parasite. In more severe forms, yellowing of the skin or eyes (jaundice) may develop, as well as digestive problems such as vomiting or diarrhea. In severe cases, seizures or changes in neurological behavior may occur, indicating involvement of the brain by the parasite.

Causes and risk factors
The disease is caused by single-celled parasites of the genus Plasmodium, specific to primates. Several species of Plasmodium can infect New World monkeys such as capuchins, some being shared with other primates. The parasite enters the bloodstream after a bite from an infected mosquito, then invades red blood cells which it progressively destroys.
Living in a geographic area where the mosquito vector is present, particularly in humid tropical environments.
Exposure to an outdoor environment not protected against mosquitoes, especially at dusk and dawn.
A weakened immune system due to stress, another illness, or very young or advanced age increases the severity of symptoms.
Transmission
Transmission occurs only through the bite of a mosquito of the genus Anopheles that has previously bitten an infected animal. There is no direct transmission from one animal to another without passage through the mosquito.
Diagnosis and care
Diagnosis is the veterinarian's responsibility, based on clinical examination and laboratory tests. Care is decided case by case.
The veterinarian suspects the disease when signs of fever and anemia appear in an animal living in or having recently stayed in a risk zone.
Diagnosis is confirmed by microscopic examination of a blood smear to visualize the parasite inside red blood cells, supplemented if needed by specialized molecular tests.
Treatment
Treatment relies on specific antimalarial medications prescribed and adjusted by the veterinarian according to the Plasmodium species identified.
General supportive care is often needed: rehydration, nutritional support, and monitoring of anemia.
Treatment depends on the cause and the animal's condition: it must be determined by a veterinarian. No self-medication.
Prevention
Prevention focuses primarily on mosquito control: netting over enclosures, insect repellents suitable for animals, and elimination of standing water around habitats.
There is no vaccine available for this disease in capuchins.
Regular health monitoring of animals in zoos or wildlife centers allows for early detection.
Possible complications
Severe anemia can develop if the parasite destroys a large number of red blood cells.
In severe forms, involvement of the brain (cerebral malaria) or internal organs can be life-threatening for the animal.

The animal has a persistent fever with no apparent cause.
The mucous membranes become pale or yellowish.
The animal becomes unusually lethargic or refuses to eat.
Convulsions or behavioral changes develop.
Transmission to humans
Certain Plasmodium species from New World monkeys have documented but rare zoonotic potential. Transmission to humans requires the presence of a competent mosquito vector; the overall risk remains low outside the tropical regions where capuchins originate.
Prognosis
The prognosis depends on how quickly treatment is started. An infection detected early and treated by a veterinarian specialized in exotic animals has a good chance of a favorable outcome. Severe cases with neurological involvement or severe anemia worsen the prognosis.
