Simian malaria is a parasitic blood disease affecting baboons and other non-human primates. It is caused by parasites of the genus Plasmodium, similar to those responsible for human malaria but usually of different species. These parasites invade the animal's red blood cells, causing their progressive destruction. The disease is transmitted through mosquito bites, mainly from Anopheles mosquitoes. In the baboon, infection may remain silent for a long time, with the animal acting as a carrier, or it may progress to marked anemia if the animal's immune defenses weaken.

Signs vary greatly depending on the Plasmodium species involved and the baboon's overall condition.
Irregular fever that comes and goes is often the first sign observed.
The animal becomes lethargic, less active, and loses appetite.
With time, anemia develops: the mucous membranes (gums, eyelids) become pale.
Progressive weight loss may appear in chronic cases.
Icterus (yellowish discoloration of mucous membranes or skin) is sometimes visible when destruction of red blood cells is substantial.
Some animals, especially healthy adults, show no visible symptoms despite the parasite's presence in the blood.

Causes and risk factors
The disease is caused by parasitic protozoans of the genus Plasmodium (for example Plasmodium cynomolgi, P. inui, or other species adapted to non-human primates depending on the region). These parasites multiply in the baboon's red blood cells after being introduced through a bite from an infected mosquito.
Living in an area where Anopheles mosquitoes are abundant, particularly near stagnant water sources, increases the risk of exposure. Young animals, weakened or immunocompromised individuals, and those kept in captivity in environments with high mosquito density are more likely to develop severe clinical disease. Stress, malnutrition, or concurrent illness can reactivate an infection that has remained silent.
Transmission
Transmission occurs through bites from Anopheles mosquitoes, which act as vectors by carrying the parasite from an infected animal to a healthy one. There is no direct transmission from one baboon to another without passage through the mosquito. Accidental transmission through blood transfusion or a contaminated needle stick is theoretically possible but rare.
Diagnosis and care
Diagnosis is the veterinarian's responsibility, based on clinical examination and laboratory tests. Care is decided case by case.
The veterinarian suspects the disease when irregular fever is associated with anemia in a baboon living in an at-risk area. Diagnosis is based on microscopic examination of a stained blood smear, which allows visualization of the parasite inside red blood cells. Molecular (PCR) or serological tests may be used additionally to identify precisely which Plasmodium species is responsible, particularly in research centers or zoos.
Treatment
Treatment relies on specific antimalarial drugs, selected by the veterinarian according to the parasite species identified and the animal's condition. Supportive care (fluids, nutritional support) may be needed if anemia is severe. No self-treatment should be attempted: choice and duration of treatment must be decided by a veterinarian.

Prevention
Mosquito control is the main measure: elimination of stagnant water, use of mosquito nets or repellents in enclosures and housing areas.

Possible complications
Severe anemia can lead to marked weakness or even collapse in serious cases. Complications affecting the spleen or liver are possible during chronic infections. In immunocompromised animals, infection can reactivate severely after a period of dormancy.

Transmission to humans
Certain Plasmodium species from non-human primates, such as Plasmodium knowlesi, can accidentally infect humans through mosquito bites, especially in regions where humans and non-human primates share the same forest habitat. The risk of direct transmission between a baboon and a person (without mosquito involvement) is considered negligible. People working in close contact with baboons in captivity (keepers, veterinarians) in mosquito-prone areas should remain vigilant.
Prognosis
The outlook is generally favourable in healthy adult animals, with infection often remaining silent or mild. It is more guarded in young animals, weakened or immunocompromised individuals, where anaemia may become severe. Early veterinary management significantly improves the chances of recovery.
