These worms settle in the fish's digestive tract, where they feed at the expense of their host. This disease affects both wild and captive scorpionfish, with varying prevalence depending on environment and diet.
Gastrointestinal nematodes belong to several families of roundworms capable of parasitizing marine fish. Their life cycle can be direct or involve an intermediate host, such as a small crustacean or mollusk consumed by the scorpionfish.

The disease is caused by nematodes (roundworms) that colonize the stomach or intestines of scorpionfish. These parasites attach to the digestive wall or live freely in the intestinal lumen, disrupting nutrient absorption.
Infestation most often occurs through ingestion of contaminated prey (small crustaceans, mollusks, other fish) that harbor infective larvae. In the wild, scorpionfish become infected by feeding on their usual prey. In captivity, the introduction of uncontrolled live prey or already-infected fish is the main source of contamination.
Scorpionfish fed live, uncontrolled prey (shrimp, wild-caught small fish) face increased risk. High stocking density in the same tank or pond also promotes transmission between individuals.
A weakened immune system—for example from chronic stress, poor water quality, or unbalanced diet—makes the scorpionfish more vulnerable to severe infestation. Recently captured or transported fish are also at greater risk due to stress-related immune suppression.

Transmission occurs mainly through ingestion of prey (crustaceans, mollusks, small fish) carrying infective larvae. A direct fish-to-fish cycle is possible for some nematode species, particularly via water contaminated with eggs.
Diagnosis and care
Diagnosis is the veterinarian's responsibility, based on clinical examination and laboratory tests. Care is decided case by case.
Diagnosis is based mainly on examination of the fish's droppings for nematode eggs or larvae, performed by a veterinarian specializing in aquatic animals. In some cases, endoscopy or post-mortem examination may reveal adult worms in the digestive tract.
Clinical observation of behavior, body condition, and appetite completes the assessment. Specialist consultation is advised as soon as suggestive signs appear, since definitive diagnosis requires tests the owner cannot perform alone.
Treatment
Treatment involves administering antiparasitic drugs specific to nematodes, usually mixed into food or added to the water following the specialist's instructions. Drug choice depends on the type of nematode identified.
Nutritional support may be combined to help the fish regain body condition after treatment. Treatment effectiveness is monitored through a repeat stool examination a few weeks after intervention.
Treatment depends on the cause and the animal's condition: it must be determined by a veterinarian. No self-medication.
Prevention
The best prevention is to avoid feeding scorpionfish live prey from uncertain sources. Frozen or prepared foods with known origin substantially reduce the risk of parasite introduction.
Routine quarantine of any new fish before adding it to a community tank allows detection and treatment of possible infestation before it spreads. Maintaining good water quality and a balanced diet also supports the fish's natural defenses.
Possible complications
Massive untreated infestation can cause intestinal obstruction, severe weight loss, and general weakness in the fish. This makes it more vulnerable to secondary opportunistic infections, bacterial or fungal.
