These worms are very common in wild marine fish, including cod consumed in Tunisia as imported or frozen product. Infestation is part of the parasite's natural life cycle, which uses crustaceans and then fish as intermediate hosts before reaching marine mammals, its definitive hosts. This condition is chiefly known to the public for its health risk to humans when eating raw or undercooked fish.

In cod itself, infestation often goes unnoticed because the fish tolerates a moderate number of worms well. In cases of heavy infestation, progressive weight loss, reduced activity, sometimes a distended belly, and less vigorous swimming may be observed. On examination after capture, small whitish or slightly spiral-coiled worms may be visible to the naked eye in the abdominal cavity, on the liver, or in the flesh, especially around the peritoneum. These worms typically measure several centimeters.

Causes and risk factors
The cause is the presence of larvae or adults of nematodes from the Anisakidae family in the fish's digestive tract and organs. Cod becomes infested by feeding on small crustaceans or other fish already carrying parasite larvae.
Wild fish are more exposed than farmed fish because the parasite's cycle depends on natural prey and marine mammals in the ecosystem. Fishing grounds rich in marine mammals (seals, cetaceans) promote parasite presence. Older, larger cod that have had more opportunities to feed on infested prey tend to harbour more worms.
Diagnosis and care
Diagnosis is the veterinarian's responsibility, based on clinical examination and laboratory tests. Care is decided case by case.
In fish, diagnosis is usually made after death through visual examination of the abdominal cavity and organs during evisceration, supplemented if needed by laboratory parasitological examination to precisely identify the nematode species. Routine diagnosis on live fish outside of surveillance programs for fishing stocks is not available.
Treatment
No antiparasitic treatment is applied to infested wild fish. Management relies on removal of organs and culling of visibly heavily parasitized fish during processing on shore, as well as on freezing or cooking processes that inactivate the larvae before human consumption.
Treatment depends on the cause and the animal's condition: it must be determined by a veterinarian. No self-medication.
Prevention
In live fish, there are no individual prevention measures applicable in extensive farming or wild-caught settings. Prevention primarily concerns human food safety: freezing at sufficiently low temperature for an appropriate duration before consuming raw or marinated fish, thorough cooking of the fish, and careful visual inspection of the flesh and organs during evisceration.
Possible complications
In cod, heavy infestation can cause chronic wasting and general weakening, but is rarely fatal in itself. The main concern is the risk of transmission to humans if infested flesh is consumed without proper freezing or cooking, which can cause anisakiasis or allergic reactions.

Humans can become infested by eating raw, marinated, or undercooked cod containing live Anisakis larvae. This can cause anisakiasis, with abdominal pain, nausea, and sometimes allergic reactions that may become severe in sensitive individuals. Proper freezing or sufficient cooking of the fish destroys the larvae and eliminates this risk.
Prognosis
The outlook for the fish itself is usually good, since the infestation is well tolerated unless the parasite burden is very high. From a public health standpoint, the outlook depends entirely on how the fish is prepared before eating.
