Like many amphibians, it may carry roundworms (nematodes) in its digestive tract. These parasites live in small numbers in many wild animals without causing visible disease. The problem arises when their numbers increase substantially, particularly in weakened animals, those under stress, or those kept in captivity in poor conditions. This is known as clinically significant gastrointestinal parasitism.

Causes and risk factors
The disease results from an excessive number of nematodes (roundworms) in the stomach or intestine of the hellbender. Ingestion of eggs or larvae present in water, sediment, or via infested prey is the most common route of contamination.
Hellbenders kept in captivity in poorly maintained tanks with poor water quality are at greater risk. High animal density in the same tank promotes transmission between individuals. A weakened immune system from stress, inappropriate water temperature, or an underlying disease increases the risk of severe parasitism. Feeding with live prey from uncontrolled sources (earthworms, fish, wild invertebrates) may introduce parasites. In the wild, degraded water quality and environmental stress are also contributing factors, as this species is already considered vulnerable.
Diagnosis and care
Diagnosis is the veterinarian's responsibility, based on clinical examination and laboratory tests. Care is decided case by case.
Diagnosis mainly relies on microscopic examination of feces to detect nematode eggs or larvae using flotation or direct examination techniques. The veterinarian may also observe worms directly in the stool or through endoscopy in more complex cases. Assessment of the animal's general condition (weight, behavior, water quality) completes the evaluation. In some cases, necropsy of a deceased animal confirms the diagnosis and identifies the specific parasite responsible.
Treatment
Treatment uses antiparasitic medications (dewormers) suited to amphibians, prescribed and dosed exclusively by a veterinarian specializing in exotic animals. The sensitivity of amphibians to certain chemicals requires great care in selecting and administering treatment. At the same time, improving husbandry conditions (water quality, stress reduction, appropriate diet) is essential to promote recovery and prevent recurrence. Nutritional support may be provided if the animal shows significant weight loss.

Prevention
Good water quality with effective filtration and stable physicochemical parameters substantially reduces the risk of parasitism. Avoiding overstocking in maintenance tanks limits transmission between individuals. It is recommended to quarantine all new animals before introducing them with other hellbenders, with parasitological examination of their feces. Prey used for feeding should ideally come from controlled sources rather than unmonitored wild habitats. Regular veterinary monitoring with fecal examination allows detection of parasitic load before the appearance of severe clinical signs.
Possible complications
Heavy untreated infestation can cause intestinal obstruction, severe nutrient malabsorption, and general weakening that opens the door to secondary infections.
