Hippopotamus pneumonia is a lung infection that affects this semi-aquatic animal, both in the wild and in zoological parks. It involves inflammation of lung tissue, most often caused by bacteria, sometimes triggered or worsened by a virus or opportunistic fungus. In hippopotamuses, this disease is particularly feared because the animal spends much of its life in water and easily masks signs of weakness, which often delays detection.

Early signs are often subtle: the animal may stay submerged longer or conversely avoid the water, eat less, and appear lethargic. Faster or labored breathing may be observed, sometimes audible from the shore, along with discharge from the nostrils. Coughing can occur, although this sign is difficult to spot in this species that is not very demonstrative. In advanced cases, the animal may show fever, isolation from the group, and marked fatigue, with sometimes difficulty getting out of the water or moving normally.

Causes and risk factors
Pneumonia most often results from infection by opportunistic bacteria present in the aquatic environment or in the animal's respiratory tract, which take advantage of weakened defenses to multiply in the lungs. Respiratory viruses or fungi can also be responsible, especially in animals whose immune defenses are compromised. Accidental inhalation of contaminated water, silt, or irritant particles can also trigger inflammation that then becomes secondarily infected.
Young animals, elderly individuals, and those weakened by stress, injury, or other illness are more vulnerable. Poor quality water that is inadequately filtered or too cold, as well as sudden temperature changes, promote the disease, especially in captivity. Transport, enclosure changes, or conflictual cohabitation can also play a role by weakening the animal.
Diagnosis and care
Diagnosis is the veterinarian's responsibility, based on clinical examination and laboratory tests. Care is decided case by case.
The veterinarian relies first on distant observation, as direct approach to a hippopotamus is risky and often requires sedation. Lung auscultation, when possible, as well as respiratory or blood samples help confirm infection and identify the causative agent. Imaging techniques such as ultrasound can supplement the examination depending on available resources at the site.
Treatment
Treatment relies mainly on antibiotic therapy tailored to the identified agent, possibly combined with anti-inflammatory drugs to reduce lung inflammation. General supportive care, including adequate hydration and appropriate nutrition, often accompanies management. In severe cases, respiratory support or specialist veterinary hospitalisation may be considered, although logistics are complex for an animal of this size.

Maintaining clean, well-filtered water at appropriate temperature is the most important measure in a controlled environment. Daily observation of behaviour, appetite and time spent out of the water allows early detection of any suspicious changes. Possible complications
Without treatment, infection can spread and cause severe respiratory failure. Septicaemia—that is, bacterial spread throughout the body via the bloodstream—is possible and life-threatening. The disease may also become chronic, weakening the animal durably and making it more susceptible to other infections.

The animal remains immobile or unusually isolated. Its breathing appears rapid, laboured or noisy. It refuses to eat or shows marked lethargy. Nasal discharge or cough appears. The animal avoids water or conversely remains abnormally submerged in it.
Prognosis
The prognosis depends greatly on how early the diagnosis is made, which is often delayed because signs can be subtle in this species.
Rapid and appropriate treatment usually leads to good recovery.
However, advanced pneumonia or pneumonia complicated by septicemia significantly worsens the prognosis.
