Tetrodotoxin is an extremely potent natural substance produced by bacteria that live in association with certain fish, particularly pufferfish of the Tetraodontidae family (such as Lagocephalus sceleratus, a species found in the Mediterranean, including off the Tunisian coast).
This toxin accumulates mainly in the liver, ovaries, intestines, and skin of the fish, whereas muscle tissue generally contains much less, but never absolutely none.
This is not an infectious disease of the fish itself: the pufferfish carries the toxin without being sick from it. The danger concerns animals and humans who eat it or handle it inappropriately.

Causes and risk factors
The cause is ingestion, chewing, or sometimes simple contact of the mouth with a pufferfish (Tetraodontidae) containing tetrodotoxin. The toxin blocks normal nerve and muscle function by preventing the electrical signals needed for muscle contraction and breathing.
The pufferfish does not produce the toxin itself: it is thought to come from bacteria present in its body and along the marine food chain. The amount of toxin varies by species, season, geographic location, and individual, making the risk completely unpredictable.
Animals living near or visiting fishing areas, ports, and beaches where pufferfish may be discarded or abandoned after capture face greater exposure.
Curious dogs and cats that explore fishing waste or find a stranded pufferfish on the shore are at particular risk.
In Tunisia and more broadly throughout the Mediterranean, the increasing presence of Lagocephalus sceleratus (an invasive species from the Red Sea) raises the likelihood of encountering this fish along coasts and in fishing catches.
Diagnosis and care
Diagnosis is the veterinarian's responsibility, based on clinical examination and laboratory tests. Care is decided case by case.
There is no routine quick test available in veterinary practice to confirm the presence of tetrodotoxin in an animal's body. Diagnosis relies mainly on the circumstances (known or suspected ingestion of pufferfish) and the symptoms observed.
The veterinarian urgently assesses the animal's breathing, ability to move, and level of consciousness. Additional tests may be performed to rule out other causes of poisoning or paralysis.
Treatment
There is no specific antidote for tetrodotoxin. Treatment consists solely of supporting vital functions while the body gradually eliminates the toxin.
This may include respiratory support, intravenous fluids to maintain circulation and hydration, and close monitoring in a specialized veterinary facility, possibly in intensive care.
Home treatment attempts are not appropriate: only a veterinarian can assess the severity of the case and implement the necessary supportive measures.
Treatment depends on the cause and the animal's condition: it must be determined by a veterinarian. No self-medication.
Prevention
The best prevention is to keep the animal away from any pufferfish—dead or alive, whole or in pieces.
Any pufferfish found on the beach or brought in by fishing activity should be disposed of immediately and safely, without leaving it accessible to animals or children.
Monitoring animals during walks on beaches and in port areas, and training them (as much as possible) not to pick up unknown marine objects or debris, helps reduce the risk.
Possible complications
The most serious complication is respiratory failure from paralysis of the breathing muscles, which can cause death if the animal is not supported in time.
Irregular heartbeat and a drop in blood pressure are also possible in severe cases.
When to see a veterinarian
The animal has bitten, chewed, or swallowed a pufferfish or its remains, even in small amounts.
The animal shows excessive drooling, vomiting, or a stumbling gait after contact with the sea or a fishing area.
The animal shows weakness that rapidly progresses to an inability to stand.
The animal has difficulty breathing or appears to be panting abnormally.
The animal shows tremors or seizures after an outing near the sea.
Transmission to humans
Tetrodotoxin is not a disease that transmits between animals and humans in the classical sense, but it is dangerous to both. A person who consumed the flesh of an improperly prepared pufferfish, or who handled an animal that had regurgitated the fish, could be exposed to the same type of toxin.
Pufferfish caught by untrained individuals should never be consumed outside of strict preparation by trained professionals—a practice that is not recommended in Tunisia anyway.
Prognosis
The prognosis depends directly on the amount of toxin ingested and how quickly veterinary treatment is provided. Without supportive care, severe poisoning can be fatal within a few hours.
With prompt respiratory support and intensive care, some animals can survive because tetrodotoxin is gradually eliminated from the body if vital functions are maintained in the meantime.
Frequently asked questions
Can a dog die from simply licking a pufferfish?
Simple licking is less dangerous than a bite or ingestion, but the toxin can also be present on the fish's skin. Any exposure, even minimal, should be taken seriously and reported to a veterinarian.
