Anisakiasis is an infestation caused by parasitic roundworm larvae from the family Anisakidae, very common in anchovies (Engraulis encrasicolus) and many other marine fish.
This parasite usually does not cause visible illness in anchovies, but it represents a major public health concern because it can infect humans when the fish is eaten raw, marinated, or undercooked.
In Tunisia, where anchovies are widely consumed especially marinated or salted, this parasitic infection is closely monitored by seafood safety authorities.

In anchovies themselves, infestation often goes unnoticed: the fish continues to swim and behave normally, even with several larvae embedded in its flesh or organs.
In heavy infestations, weakening of the fish or reduced swimming activity may occasionally be observed, but these signs remain uncommon and non-specific.
The most obvious sign for consumers is the discovery, during gutting or filleting, of small whitish or brownish worms coiled in a spiral, visible to the naked eye in the abdominal cavity or within the fillets.

Causes and risk factors
The infection is caused by nematode larvae from the genus Anisakis (and sometimes Pseudoterranova or Contracaecum). These worms have a complex life cycle involving several hosts.
Eggs are released into seawater through the faeces of marine mammals (dolphins, seals, cetaceans), which are the definitive hosts of the adult parasite. Larvae hatch and are ingested by small crustaceans, which are in turn eaten by anchovies and other fish, making them intermediate or paratenic hosts.
In anchovies, the larva migrates from the digestive tract to the muscles and organs, where it becomes encysted without causing serious disease in the fish.
Anchovies caught in the Mediterranean, including off Tunisian coasts, show variable infestation rates depending on season, fishing area, and fish size (larger and older individuals are often more heavily infected).
Proximity to areas frequented by marine mammals increases the risk. Anchovies caught offshore or in deeper waters may have different infestation rates than those caught near the coast.
Diagnosis and care
Diagnosis is the veterinarian's responsibility, based on clinical examination and laboratory tests. Care is decided case by case.
There is no individual veterinary diagnosis for anisakiosis in anchovies, as this species is not monitored as a pet or farmed animal.
Monitoring occurs at the batch level, through visual inspection of the viscera and flesh during processing at the market or factory, sometimes supplemented by transillumination techniques (light-candling) or laboratory analysis to detect larvae.
Treatment
There is no antiparasitic treatment applicable to live or caught anchovies to eliminate Anisakis larvae.
The only possible intervention is destruction of larvae in the food product through freezing or cooking, which falls under food safety standards rather than veterinary treatment of the animal.
Treatment depends on the cause and the animal's condition: it must be determined by a veterinarian. No self-medication.
Prevention
There is no way to prevent infestation of anchovies in their natural environment: this is a natural parasitic phenomenon in marine ecosystems.
Prevention focuses mainly on human food safety: freezing at sufficiently low temperature for an appropriate length of time before raw or marinated consumption, thorough cooking of the fish, and prompt evisceration after capture to limit larval migration to the muscle tissue.
Fishing and processing professionals apply visual inspection (candling) and follow the sanitary freezing standards set out in regulations.
Possible complications
Direct complications in anchovies are rare and poorly documented, as infestation is generally well tolerated by the fish.
In humans who consume untreated infested fish, possible complications include acute abdominal pain, vomiting, and sometimes allergic reactions that may become severe in sensitized individuals.
When to see a veterinarian
Individual veterinary consultation is not relevant for an anchovy.
Conversely, a human healthcare provider should be consulted promptly if abdominal pain, vomiting, or signs of allergy appear after eating raw or undercooked fish.
Any concern about the sanitary compliance of a batch of fish should be reported to food control authorities.
Transmission to humans
Anisakiosis is a recognized food-borne zoonosis: humans become infected by eating raw, marinated, cold-smoked, or undercooked fish containing live Anisakis larvae.
The larvae do not develop into adults in humans, who are accidental hosts, but they can cause acute digestive pain and allergic reactions, sometimes severe.
Freezing at regulatory temperature for an adequate duration or thorough cooking of the fish destroys larvae and eliminates the risk.
Prognosis
Prognosis for the anchovy itself is generally good, as infestation does not significantly affect its survival or breeding in most cases.
For humans, prognosis depends on management: most cases of human anisakiasis respond well to medical treatment, though more severe forms may require endoscopic or surgical intervention.
