The signs are often discreet at first because the meerkat naturally hides its weakness. We may observe unusual fatigue, an animal that stays away from the group, a loss of enthusiasm during digging or playing activities. \n\nWith progression, shortness of breath appears even at rest, faster or more labored breathing, sometimes a cough. The stomach may appear swollen if there is fluid accumulation in the abdomen. The gums may become pale. A loss of appetite and progressive weight loss often accompany the disease. In advanced forms, sudden collapse or sudden death can occur without clear warning signs.

Causes and risk factors
In the majority of cases reported in meerkats, the exact cause remains poorly identified (we speak of so-called “primitive” or idiopathic Cardiomyopathy). Abnormal deposits in the heart muscle, chronic inflammation or a hereditary component are suspected according to studies carried out in captivity. Nutritional deficiencies, chronic infectious diseases or premature aging linked to captivity conditions can also play a contributing role.
Advanced age is the main risk factor, with the disease mainly affecting meerkats over 5-6 years old. Life in captivity appears to be associated with a higher frequency, possibly linked to chronic stress, diet or lack of exercise. A genetic predisposition within certain captive lines is also mentioned.
Diagnosis and care
Diagnosis is the veterinarian's responsibility, based on clinical examination and laboratory tests. Care is decided case by case.
The veterinarian relies on cardiac and pulmonary auscultation, supplemented if possible by a cardiac ultrasound which allows visualization of the muscle and chambers of the heart in movement. Chest and abdominal x-rays, an electrocardiogram and blood tests can complete the assessment. These examinations often require appropriate restraint or even light sedation given the lively temperament of the species.
Treatment
Treatment aims to support cardiac function and limit fluid accumulation: diuretics, cardiotonic drugs or heart rate regulators, and sometimes vasodilators, depending on the type of Cardiomyopathy identified by the veterinarian. Relative rest and careful diet management accompany medical care.
Treatment depends on the cause and the animal's condition: it must be determined by a veterinarian. No self-medication.
Prevention
There is no known specific prevention due to lack of a single identified cause. A balanced diet adapted to the species, an enriched environment allowing physical exercise and the reduction of chronic stress are generally recommended. Regular veterinary monitoring with cardiac examination in older animals allows earlier detection.
Possible complications
The disease can progress to congestive heart failure with fluid accumulation in the chest or abdomen. Heart rhythm disturbances, episodes of syncope (brief loss of consciousness) and a risk of sudden death are possible at any stage, including in an apparently stable animal.

Unusual shortness of breath, even slight or after moderate exercise.
Persistent fatigue or refusal to participate in usual group activities.
Stomach that seems abnormally swollen.
Pale or bluish gums.

Prognosis
The prognosis is poor, as Cardiomyopathy is a progressive and generally incurable disease. Early diagnosis and regular cardiological monitoring sometimes help slow progression and improve comfort of life, but the disease remains a frequent cause of death in elderly meerkats in captivity.
