Gastrointestinal parasitism in harbour porpoises is an infestation of the stomach and intestines by roundworms (nematodes), mainly species related to Anisakidae such as Anisakis simplex and Pseudoterranova. These parasites are very common in wild porpoises, including those in apparent good health. They become a genuine health problem when present in large numbers or when the animal is already weakened by other factors (pollution, food shortage, stress). The harbour porpoise (Phocoena phocoena) is a small coastal cetacean species particularly studied for this type of parasitism during post-mortem examinations of stranded animals.

In living animals, signs are difficult to observe since the harbour porpoise lives at sea and is only seen at close range when stranded or in distress.
Progressive weight loss, apparent loss of appetite, and general weakness may occur.
Vomiting or digestive upset can develop during heavy infestations.
In severe cases, worms can cause inflammation of the stomach or intestinal wall, sometimes with ulcer or nodule formation.

Causes and risk factors
Infestation is caused by nematodes (roundworms) whose life cycle involves multiple intermediate hosts: crustaceans, then fish or cephalopods, which are subsequently eaten by the harbour porpoise, the definitive host in which the worms mature into adults in the stomach or intestines.
Feeding on fish with high parasite loads, a weakened immune system from stress, pollution, or concurrent illness, as well as young age or poor body condition increase the risk of severe parasitic infection.
Transmission
Harbour porpoises become infected by eating fish or cephalopods carrying worm larvae. There is no direct transmission between porpoises.
Diagnosis and care
Diagnosis is the veterinarian's responsibility, based on clinical examination and laboratory tests. Care is decided case by case.
Diagnosis in living animals is very difficult and relies mainly on clinical observation during exceptional care (stranding, rescue). Definitive diagnosis most often comes after death, during necropsy which allows direct visualization of worms in the stomach or intestines and assessment of associated tissue damage. Fecal analysis may sometimes reveal parasite eggs in an animal that has been captured or is receiving care.
Treatment
In a porpoise receiving care at a rehabilitation center, antiparasitic treatment adapted for marine mammals may be considered by a veterinarian specializing in marine animals, along with supportive care (rehydration, nutritional support). In the wild, no treatment is possible.
Treatment depends on the cause and the animal's condition: it must be determined by a veterinarian. No self-medication.
Prevention
Prevention is not possible in wild populations of this species. Preserving the quality of coastal waters and fish prey populations indirectly helps limit stress and immune compromise in porpoises, making them less vulnerable to the effects of parasitic disease.
Possible complications
A heavy parasite load can cause severe inflammation, ulcers, or even perforation of the digestive tract wall. It can also worsen general weakness and contribute, combined with other factors, to the animal's death.
