Cetacean Morbillivirus is a serious viral disease that affects marine mammals, including narwhals. This virus belongs to the same family as that of distemper in dogs or measles in humans. It causes respiratory, neurological and immune damage that is often severe. Episodes of mass mortality have been observed in several species of cetaceans around the world, although documented cases specifically in the narwhal remain rare due to its isolated Arctic habitat.

Affected animals may show marked depression, loss of appetite and progressive weight loss. Respiratory problems often appear, with difficult or irregular breathing when returning to the surface. Neurologically, abnormal movements, disorientation, dizziness or convulsions may be observed. The eyes may be red or have discharge. As the immune system is weakened by the virus, secondary infections (bacterial, parasitic or fungal) frequently complicate the clinical picture.

Causes and risk factors
The disease is caused by a morbillivirus specific to cetaceans, an RNA virus close to those responsible for rinderpest, distemper and measles. There are several strains depending on the regions and species of cetaceans affected (dolphin morbillivirus, porpoise morbillivirus in particular). The virus primarily attacks cells of the immune system and the central nervous system.
Proximity and close social contact between individuals favor the spread, with narwhals often living in groups. A weakened immune system, young age or poor body condition increase susceptibility to infection. Climate change modifying sea ice zones and the movements of cetacean populations could encourage new contacts between species and the spread of the virus towards Arctic populations that were previously little exposed.
Transmission
Transmission occurs mainly by direct contact between animals, via respiratory secretions (aerosols during surface breathing) and body fluids. The virus can also be transmitted through contact with infected animal carcasses. It is not transmitted to humans.
Diagnosis and care
Diagnosis is the veterinarian's responsibility, based on clinical examination and laboratory tests. Care is decided case by case.
The diagnosis is based on laboratory analyzes carried out from post-mortem samples (lung, brain, lymph node tissues) or biological samples (blood, secretions) from a still living animal if accessible. Techniques used include detection of viral genetic material (PCR), histopathology looking for characteristic lesions, and serology to detect antibodies. Given the narwhal's lifestyle, in vivo diagnosis is extremely difficult; most confirmations are made on stranded or dead animals.
Treatment
There is no specific antiviral treatment validated for this disease in wild cetaceans. Care, when possible (exceptional cases of accessible animals in distress), remains mainly supportive care and the treatment of secondary infections with antibiotics or antiparasitics according to the needs identified by a veterinarian specializing in wildlife.

Prevention
There is no direct means of prevention applicable in the wild for the narwhal. Epidemiological surveillance of Arctic cetacean populations, the study of carcasses found and the monitoring of strandings make it possible to detect viral circulation early. Limiting stress of human origin (maritime traffic, pollution) and preserving habitat indirectly contributes to maintaining healthier, and therefore more resilient, populations.
Possible complications
The main complications are secondary bacterial or parasitic infections favored by the immunosuppression caused by the virus. Severe and irreversible neurological damage may occur. In certain species of cetaceans, the disease has been associated with episodes of mass mortality affecting several hundred individuals in a few weeks.

In other better-studied cetacean species, the mortality observed during epidemic episodes may have been very high. Some animals nevertheless develop immunity and survive, but precise data are lacking for the narwhal in particular due to the rarity of documented cases.
