Gastrointestinal parasitism in red mullet refers to infestation of the digestive system of this marine fish by roundworms, known as nematodes. These parasites live in the intestine, stomach, or sometimes the abdominal cavity of the red mullet. It is a natural and widespread occurrence in wild fish of the Mediterranean, including along Tunisian coasts. The vast majority of red mullets caught at sea carry a parasite load, often without apparent ill effects to the fish itself.
This fact sheet concerns mainly wild red mullet. In rare cases of farming or captivity (marine aquariums, research facilities), the same type of parasitism may occur but remains poorly documented.

In living fish, the signs are often subtle and difficult to observe at sea or after catch.
A heavily parasitized fish may appear thinner than normal, with less muscular body tissue. Slowed growth is possible with severe and prolonged infestation.
The most obvious sign becomes apparent when the fish is opened: small whitish or slightly coiled roundworms can be seen in the intestine, stomach, or lodged against the abdominal cavity wall. They are sometimes visible to the naked eye during culinary preparation of the fish.
In most cases, the fish shows no external signs and parasitism is discovered only when gutting the fish.

Causes and risk factors
The roundworms responsible belong mainly to marine nematode families such as Anisakis and Hysterothylacium. Red mullet becomes infected by feeding on small invertebrates or prey themselves carrying parasitic larvae.
These nematodes have a complex life cycle involving several successive hosts: small crustaceans, then fish such as red mullet that serve as intermediate or paratenic hosts, and finally marine mammals or sometimes seabirds in which the parasite becomes adult and reproduces.
Red mullet living in coastal areas rich in infested prey are at higher risk. Fish size and age play a role: older and larger individuals, having had more time to accumulate parasites through feeding, typically carry higher parasite loads.
Season and fishing grounds can influence the level of infestation, as can proximity to areas frequented by marine mammals, which are the definitive hosts for some of these parasites.
Transmission
Transmission occurs through feeding within the marine food chain: red mullet becomes infected by consuming prey (small crustaceans or other invertebrates) carrying nematode larvae. There is no direct transmission between fish.
Diagnosis and care
Diagnosis is the veterinarian's responsibility, based on clinical examination and laboratory tests. Care is decided case by case.
There is no individual veterinary diagnosis for a wild fish destined for fishing. Parasites are generally identified during sanitary inspection of fish products through visual examination of the flesh and organs, sometimes completed by candling (transillumination) of fillets in a laboratory or processing facility.
In an aquarium or research setting, examination by a veterinarian specializing in aquatic animals may include fecal analysis or necropsy.
Treatment
There is no antiparasitic treatment applied to wild fish at sea. Management occurs at the product level after capture, through inspection and removal of visibly affected parts, or by physical treatments such as freezing, which inactivates larvae.
In captivity, a specialized veterinarian may consider antiparasitic drugs suited to fish, but this remains rare and regulated.
Treatment depends on the cause and the animal's condition: it must be determined by a veterinarian. No self-medication.
Prevention
There is no preventive measure applicable to wild fish, as this phenomenon is part of the natural marine cycle.
For the consumer, prevention relies on proper fish preparation: quick evisceration after catch, visual inspection of the abdominal cavity and fillets, thorough cooking or prior freezing if raw or undercooked consumption is planned.
Possible complications
In the red mullet itself, infestation is usually well tolerated, except in cases of very high parasite loads that may affect overall condition and growth.
The main concern relates to human health: certain nematodes such as Anisakis can, if ingested alive by humans through raw or undercooked fish, cause anisakiasis, a digestive condition sometimes accompanied by allergic reactions.
When to see a veterinarian
A fishing or sanitary control professional should be alerted if an abnormally high number of worms is observed in a batch of fish.
Fish showing abnormally altered flesh, very numerous worms, or worms visible in fillets should be removed from consumption.
If digestive symptoms occur after consuming raw or undercooked fish (abdominal pain, vomiting, allergic reactions), consult a physician promptly.
Prognosis
For the red mullet itself, the outlook is generally good: most fish live normally despite intestinal parasites, except in cases of extreme infestation.
For food safety, the risk is manageable through proper preparation and cooking of the fish.

Is the red mullet I buy necessarily parasitized?
A large proportion of wild red mullet carry roundworms in their viscera, which is a very common natural phenomenon in the Mediterranean. This does not make the fish unfit for consumption if the fish is properly prepared and cooked.
Can you eat a red mullet that contains worms?
After evisceration and sufficient cooking, or after prior freezing for raw consumption, the risk is eliminated. It is recommended to remove the viscera quickly and inspect the flesh.
Are these worms dangerous for the fish itself?
