Ulcerative stomatitis is an infection of the turtle's mouth that causes sores (ulcers) on the tongue, palate and gums.
It frequently affects terrestrial turtles such as the Greek tortoise, particularly when their living conditions are not optimal.
This disease, sometimes called “rotten mouth” by breeders, is a veterinary emergency because it prevents the animal from eating properly.

We observe whitish, yellowish or caseous deposits (resembling cottage cheese) inside the turtle's mouth.
Redness, sores or small ulcers appear on the tongue, palate or gums.
The turtle may drool excessively and exhibit characteristic bad breath.
She eats less, or even more at all, and gradually becomes listless and emaciated.
In advanced cases, swelling of the jaw or head may be visible, as well as difficulty closing the mouth completely.

Causes and risk factors
Ulcerative stomatitis most often results from an opportunistic bacterial infection, particularly by bacteria of the Pseudomonas genus or other germs naturally present in the turtle's mouth.
These bacteria take advantage of a weakening of the animal's immune system to proliferate and cause lesions.
Viral (herpesvirus) or fungal infections can also be the cause, sometimes in association with bacteria.
Oral trauma, vitamin A deficiency or unsuitable living conditions are often the starting point of the disease.
Turtles recently emerged from hibernation, weakened or poorly nourished are particularly at risk.
An unsuitable environment (too low temperature, absence of UV rays, excessive or insufficient humidity) favors the appearance of the disease.
Stress, overcrowding in an enclosure and contact with already sick turtles increase the risk of contamination.
A vitamin A deficiency, common in turtles fed an unbalanced diet, weakens the oral mucous membranes.
Transmission
Transmission between turtles can occur by direct contact, notably via saliva, or indirectly by shared equipment or food when viral agents are involved.
Diagnosis and care
Diagnosis is the veterinarian's responsibility, based on clinical examination and laboratory tests. Care is decided case by case.
The exotic animal veterinarian examines the turtle's mouth using a light source, after gently opening the jaws. He looks for whitish or yellowish deposits, redness and ulcers on the tongue, palate and gums.
A swab from the lesions allows bacteriological or mycological analysis in the laboratory to identify the responsible germ.
X-rays or a CT scan may be ordered if bone damage to the jaw is suspected.
A blood test completes the assessment to assess the general condition and look for other concomitant diseases, particularly respiratory ones.
Treatment
Treatment is generally based on targeted antibiotic therapy after identification of the germ, associated with regular local cleaning of the lesions by the veterinarian.
Antifungal treatment may be necessary in the event of an associated mycological infection.
Vitamin A supplementation and nutritional support (forced feeding or fluids if necessary) are often essential until the turtle regains its appetite.
Correcting accommodation conditions (temperature, UV, hygrometry) is an integral part of the treatment.
Treatment depends on the cause and the animal's condition: it must be determined by a veterinarian. No self-medication.
Prevention
Maintaining suitable living conditions is the best prevention: correct temperature and humidity in the enclosure or terrarium, sufficient exposure to UV rays.
A varied and balanced diet, rich in fiber and natural vitamin A (green vegetables, edible wild plants), strengthens the turtle's immune defenses.
Avoid any sharp or abrasive objects in the environment that could injure the animal's mouth.
Well-prepared and monitored hibernation, reserved for healthy turtles, limits the risk of immune weakening.
Quarantining any new turtles before introducing them to other animals is also recommended.
Possible complications
Without treatment, the infection can spread to the jaw bones (osteomyelitis), making recovery much more difficult.
The turtle may stop eating because of the pain, leading to severe weight loss and general weakness.
The infection can spread to the respiratory or digestive tract, or even become generalized (sepsis), which puts the animal's life in danger.
Dehydration secondary to refusal to drink and eat worsens the prognosis.
When to see a veterinarian
The turtle has whitish or yellowish deposits in the mouth.
She has refused to eat for several days.
She drools unusually or has bad breath.
Swelling of the head or jaw is visible.
The animal appears dejected, lethargic or emaciated.
Prognosis
The prognosis is favorable if the disease is treated early, before the animal has stopped eating and before any bone damage.
In the event of late diagnosis or complications (osteomyelitis, septicemia), the prognosis becomes guarded and recovery takes longer and is uncertain.
Regular veterinary monitoring until the lesions have completely healed is necessary to avoid relapses.
Frequently asked questions
Is ulcerative stomatitis contagious between turtles?
Certain infectious agents involved, notably viruses, can be transmitted between turtles, especially in the event of close contact or shared equipment. It is therefore prudent to isolate an affected turtle during treatment.
My turtle has a small white spot in its mouth, should I be worried?
Any visible abnormality in the mouth warrants a quick consultation. The earlier the treatment is received, the simpler and more effective the treatment.
Can I clean my turtle's mouth with disinfectant myself?
You should not attempt to treat this condition alone. Some products are irritating or toxic to turtles, and a poor diagnosis delays appropriate treatment.
What are the causes of immune weakening favoring this disease?
Poorly managed hibernation, unsuitable temperature or humidity, poor nutrition or chronic stress are the main factors that weaken the turtle's defenses.
